Trial root canal needed on 47?

I have severe lower jaw pain near the ear and gum pain on 47 since 2020. I also had pain in Tooth 46 which was painful for tapping, biting etc.  Cbct scans were normal and xrays too.  Had root canal as endo suspect that tooth 46 could be reason for jaw pain. Had RCT in 2020. No relief. Had retreatRCT 3 times after that. No changes in pain and pulled that tooth in 2022 July.  But no changes in jaw pain and gum pain even after pulling 46. Gums of 47 is more painful and just throbbing in the area surrounding that tooth. The gum margin on distal and back side of tooth  is so painful and puffy. Did deep cleaning recently on this 47 as this always get tartar buildup very very often. I have severe pain in the gum and lower jaw. Gum pain in distal and back side of 47. Went to new Endo today. Did CBCT, xrays.She said cbct doesn't show infection.Suggested RCT to see if helps. She did cold test, was normal. Tapping and biting caused medium pressure pain on 47. do I need RCT ? Will it help?

To diagnose the cause of pain is not possible in cases like you over such platforms as you give a history of tooth extraction also! Many things need to be answered and examined in detail. The cause could, in fact, be non-odontogenic also. It could be referred pain from TMJ or ear. There seems to be not much problem with you tooth. Happy to see a bitewing X-ray. Since you mentioned you had pain on biting, get a fracture ruled out in CBCT. Also, get the opposite quadrant examined.

It is very difficult to say without seeing you clinically. X-rays do not suggest a requirement for RCT. You may need evaluation for trigeminal neuralgia. Also get your upper teeth evaluated.
