Hello sir, Before orthodontic treatment opg and later cephelogram are taken. If their is severe crowding is there and no space is there then extraction of premolars are done and then treatment is started.

If your teeth are very crowded or proclined outwards then teeth removal is important for achieving good results, pushing teeth back and gaining proper lip competency/seal. In such case, If he does not do the extractions then the doctor won't get enough space to align your teeth nicely.

During orthodontic treatment, if there is lack of space for alignment of your teeth, then extraction of your four teeth will be done just to gain space... Don't have to worry about it...
