This concern is a known issue with fillings if not done right. There is something called Composite Shrinkage which may cause pain. You get Immediate relief once the filling is removed. Usually if the filling is re done it should resolve the issue.
With regards to the gum swelling, there are multiple factors to assess.
1. Is there pus exudate from the Swelling ?
2. What is the location of the swelling ?
3. Is it generalized around the tooth area ?
4. Does the X ray also show any signs of infection ?
Based on all these factors, you can select antibiotics to resolve infection, if that is the reason. Or Maybe get an assessment from a Gum Specialist.
The concern here could be a combination of Gum related issues and Filling related. Both of which can be treated separately with good outcome and prognosis.
