My wife is 23 weeks pregnant and in her 22nd week anomaly scan we got this update - Impression : Single gestation corresponding to a gestational age of 22 Weeks 1 Day Gestational age assigned as per LMP Placenta - Anterior Presentation - Variable Liquor - Adequate Estimated fetal weight according to BPD,HC,AC,FL :- 554 + / - 55.4 gms. BILATERAL HYDRONEPHROSIS WITH LOWER URINARY TRACT OBSTRUCTION - SUGGESTIVE OF POSTERIOR URETHRAL VALVE - Suggested peadiatric urologist consultation Right renal pelvis 8.4mm Left renal pelvis 6.9mm SDP = 5.1 Please suggest we are super worried

Your baby’s kidneys are showing mild–moderate hydronephrosis (right 8.4 mm, left 6.9 mm), most likely due to PUJ obstruction. This is a common finding and usually improves on its own. Nothing needs to be done right now. We will repeat the ultrasound at 28 weeks and again around 32 weeks to monitor it. Your amniotic fluid and baby’s growth are important, and if those stay normal, there is no risk to the baby during pregnancy. After birth, the baby will get a kidney ultrasound to check again, and most babies do not need surgery.

Consult a fetal medicine specialist and paediatric urologist as one kidney falls under moderate and other is in mild range. Serial ultrasounds are required to see for progression of hydronephrosis. Genetic evaluation is also needed. 50% of the cases require surgery.
