Shoulder Dislocations
I had shoulder (right) dislocations 4 times. The recent one was in June, 2016. I took MRI in July, 2016 and the report says that "Hill Sach's defect with anteroinferior labral tear and inferomedially displaced (fused) bony bankart lesion favour shoulder dislocation". Doctor-1 whom i visited in Mumbai suggested me to go for an operation where there will be an open surgery stitching the torn larbal. I recently shifted to another city, here i went for final opinion where the doctor said that anthroscopic operation is required where three screws will be inserted and stitched to the labral

Shoulder joint is one of mobile joint of body at the cost of stability.
You have two options to avoid 5th dislocation.
1.soft tissue repair of shoulder joint
Or
2.bony augmentation of glenoid part for stabilty.

The bankart lesion is fused, so it needs to be mobilized. The hill Sach lesion may or may not need to be addressed depending on the size. Small lesion may be left alone, but large one cannot. So identification of Hill Sach lesion will decide further course. If it is small, then arthroscopy, otherwise an open surgery

The amount of bone loss need to be assessed by a ct scan for both glenoid and Humerus head. Only then the decision between bony procedure or soft tissue repair can be taken.
More so, if soft tissue repair is planned, it should be done arthroscopic
