Severe pain in molar teeth.
She has high sensitivity and severe pain in right upper and lower molar teeth. When the upper molar teeth is touched by the tongue, a severe pain is felt in the root as well as it radiates upto forehead.
Note:-She uses pin to clean her teeth whenever non veg is taken.

Looks like either due to repeated food lodgement she has developed a huge cavity which has gone to her pulp chamber needing a root canal another faint possibility is something is stuck between teeth & hasn’t been removed causing periodontal pain.
Next Steps
She should try n clean the area to make sure that nothing is stuck and using interdental cleaning aids make sure that she removes all the stuff lodged there.
If that doesn’t help she should put a small cotton ball dipped in clove oil there. If that also doesn’t help start medications like antibiotics & analgesics.

This scenario presented gives a probable image of a severely decayed tooth.
In this if an immediate dental visit is not considered than it might jeopardize the life of teeth as well.
As history of BP is there hence pain will be excruciating in this case.
Tab ketorol DT (dissolve the tab in quarter glass of water and drink) twice a day

Never use pin to clean interdental part use floss
X ray is required for further treatment
For now if she is nt allergic to any med or doesn't have any other medical prob then take phexin 650 twice a day
Metrogyl 400mg thrice a day
Pantop 20 once in empty stomach for 5 days
Next Steps
Gargle with warm salt water for 3-4times a day
Avoid chewing from that side

Sir please inform her , she should never use any objet to probe her teeth . That pin might have initiated infection hence the pain and fever . But Donto woory get these medicines.
-Tab Amoxyclav 625
1 tab bd x 5 days
-Tab Zerodol SP
1 tab td x 5 days
-Tab Metrogyl ER
1 tab Bd x 5 days
-Cap Pan D .
1 Cap.bd before meal x 5 days.
Let me know if any discomfort . Warm saline garglw 3-4 times a day . The tooth might need extraction after lockdown.

Kindly ask her not to use pin as it would damage her gums..for the pain if she is not allergic to any medicine ask her to take
Tab zerodol p twice daily for three days
Tab augmentin 625 mg twice daily for three days
Tab vizylac twice daily for three days
In case of severe take 2 tab of Ketorol dt sos
Disslove it in half glass of water and drink it.
