Severe itchy rashes on legs

Hi Sir/Mam, I'm facing problem with severe rashes on my right leg thighs, slowly it's spreading to left leg also. The rashes are itchy sometimes and I can't control myself from rubbing it. When I rub blood is coming out of it and slowly its healing and turning black. I consulted doctor in Hyderabad but my issues is not resolved. I'm a mom to 1 year old child, suggest me what is this and how to be treated. Attaching image.

You skin is dry and very much excoriated . Use moisturizer twice also use tacrolimus .01% oint twice
