I am attaching xray.please advise is thete any infection in my first molar.if yes then how much deep

There is a cavity inside and must have got the infection till the roots. This tooth definitely needs root canal treatment and post and core buildup and followed by one decent cap.

According to X-ray,yes it might require Root canal treatment but need to evaluate it clinically.There is no need to extract the tooth,as the tooth can be saved. You should get it treated now irrespective of how much the infection is.

As your dentist told you it's pretty deep upto if not into pulp , so as  your doctor advised either go for RCT and crown / go for tooth extraction.

Hi, The tooth decay is deep and needs to be treated. Are you having any pain? From the X-ray shown, it is better to undergo Root canal treatment followed by Crown.
