Rectal problem
Previously 2 yr ago my wife got fissure in ano that recover completely with two skin tags but now from last 10 days she got a new lump/pile/fissure or something other that cause pain during stool.She take cremaffin syrup 15 ml and polyethylene glycol 1 spoon at night since from last 7 days but still the motion is hard and dry.please suggest some remedy or medecines.please look at the image and suggest.Is it pile or fissure.

History and the photograph suggests that it is a "prolapsing internal pile".
Patient needs further evaluation by proctoscopy.
High roughage diet,regular aerobic exercise, will be helpful.
further evaluation for constipation is needed which is a root cause of piles.

It looks like she has visible hemorrhoidal tissue with multiple skin tags or a partial mucosal prolapse with a throbosed external hemorrhoid
If there is pain at a single swelling seen in the picture it might be a thrombosed external hemorrhoid
If not she might be suffering from chronic fissure in ano.

It's External Hemorrhoids.
