Your episodic symptoms — pulse spikes, breathlessness, vertigo, tingling in limbs, chest discomfort, and panic lasting 30-60 mins then fully resolving with normal ECG and cardiac markers — are very characteristic of Panic Attacks with an anxiety component. However, your investigations reveal contributing factors: microcytic anemia (Hb 12) can cause palpitations and breathlessness; elevated homocysteine (15.2) suggests B12/folate deficiency which causes nerve symptoms like the tingling in arms/legs; and mildly elevated hs-CRP indicates low-grade inflammation. Addressing these deficiencies may significantly reduce episode frequency and severity.
Next Steps
Get serum ferritin, B12, folate, and thyroid (TSH) done. See a physician to address the anemia and homocysteine. For the panic episodes, a psychiatrist or cardiologist (for a Holter monitor) consultation would be valuable. Please consult me directly on Practo for a detailed evaluation.

Based on your history and normal ECGs, a heart attack is less likely. Your symptoms could be related to anxiety, panic episodes, or an intermittent heart rhythm problem that may not appear on a routine ECG.
Next Steps
I would recommend consulting a cardiologist for a 24–48 hour Holter monitor and an echocardiogram.
Health Tips
Also, your low hemoglobin should be evaluated, as anemia can contribute to palpitations. If you develop severe chest pain, fainting, or persistent breathlessness, seek emergency care immediately.

Your heart rate is obviously going above when you are having palpitations, vertigo, shortness of breath. First you should rule out cardiac conditions.  It would have been nice if you have attached your ECG. Have you checked your BP when you had symptoms??? Conditions that may cause your symptoms 1. Cardiac - unstable paroxysmal SVT or AF 2. High BP or fall in BP with abnormal heart rythms 3. Thyroid issues 4. Panic attacks 5. Electrolyte abnormalities 6. Drug or alcohol abuse The list continues.... Better to evaluate yourself with a cardiologist
Next Steps
take remedies for high cholesterol

It is frightening to experience those pulse spikes and physical sensations, but the reports suggest your heart health is stable for now. However, we do see factors like high cholesterol, inflammation markers, and signs of potential anemia that may be contributing to how you feel. These episodes are draining, and you deserve a clear plan rather than just enduring them. I would like to review your history in detail. Please book a consultation so we can address this.
Next Steps
Your lab results indicate lipid and blood profile abnormalities that require clinical correlation to explain these recurrent episodes. Please book a consultation so we can review these findings together and create a comprehensive plan to address your health.
Health Tips
Minimise the intake of caffeine, prioritise nutrition

As per the details mentioned above, chances of any heart conditions pretty low but since your body weight is on a higher side and your lipid profile suggests high triglycerides , I would suggest you to modify your life style by making some changes to your diet and indulge in some activities so as to help you lower your body weight. While the blood picture and symptoms don’t necessarily suggest a physical cause of worry, I would like to know how you are doing mentally.
