Small perianal abscess can heal with medication. Chance for it develop into fistula depends on location of abscess, size of abscess  and whether it has healed completely. Can help you out with treatment plan

Yes, it is possible not to have a fistula even after two perianal abscess episodes. Since both were small, resolved quickly with antibiotics, did not require drainage, and you have been symptom-free for 10 months, the likelihood of an underlying fistula is low at present. Many fistulas develop after inadequately drained or persistent abscesses, which does not seem to be the case here.

Fistula usually presents with persistent or recurrent symptoms such as pain, swelling, discharge of pus, or repeated abscess formation. Since you have none of these symptoms for a prolonged period, there is no current evidence to suggest a fistula.
