Pain in lower back on left side and leg

I feel pain in my left lower back in left and left leg and some slight pain in back whenever I press my left side in scrotum region. Also 4 year back I had fistula in ano in groin and had surgery to remove it. I apply ice in back the pain settles down and reappear after prolonged sitting. Should I consultant a orthopedic or general surgeon or some other doc?
