Your father's BP is high because his kidneys are not functioning properly causing: 1. Protein loss. 2. Uncontrolled BP. 3. Breathlessness which can be related to water retention due to decreased excretion of cardiac failure due to high BP. 4. Urea and creatinine are high. 5. Potassium and Chloride are high.
Next Steps
Coversyl causes high potassium. Consult a nephrologist after 10 days because he has just started Prazopress, Nefrosave and Sodium bicarbonate.

A BP of 180–185/80 mmHg with breathlessness and 3+ protein in urine is concerning and needs prompt evaluation by his treating physician or at a hospital. Uncontrolled hypertension, heart disease, or kidney involvement may be contributing to his symptoms.
Next Steps
Feel free to consult for further evaluation and treatment plan.

This is a concerning clinical picture that needs urgent in-person attention. Urine protein 3+ combined with BP still at 180/85 despite three antihypertensive medications, and breathlessness after just 2 minutes of walking, suggests significant kidney involvement with possible fluid retention or early heart failure — both of which are interconnected at this age. The kidneys may be leaking protein due to longstanding hypertensive damage, and uncontrolled BP further worsens kidney function in a cycle. Prazopress and Nefrosave being added only 10 days ago means the dose adjustments haven't had full effect yet.
Next Steps
Your father needs to be seen by a nephrologist or physician in person urgently — blood tests for kidney function (creatinine, urea, eGFR), urine protein quantification (24-hour urine protein), and an echocardiogram to check heart function are essential next steps. Monitor for leg swelling, worsening breathlessness, or reduced urine output. Please consult me directly on Practo for a detailed evaluation.
