Need complete detailed history. May he it's due to Anxiety only. I would suggest you to stop Telmikind 40 and start Amlodipine 5mg for hypertension only after your cardiologist prescription. Amlodipine is useful if vasospasm component.

In Angina we use same medication which help you maintain your BP...and first choice in Nitrate drugs...You have to control your BP...if it is not controlled then we need to switch to other drugs...which also help you in Angina.. Consult for better and further treatment

Microvascular angina (also called Cardiac Syndrome X or INOCA – Ischemia with Non-Obstructive Coronary Arteries) means: • Angiography is normal • Major arteries are open • But small coronary vessels (microcirculation) do not dilate properly This causes: • Chest pain • Tightness • Effort intolerance Even though lipid profile and angiogram are normal. Since you are already on Telmikind 40 (Telmisartan) for hypertension, BP control is good — which is important. Microvascular angina is real, but it is usually not life-threatening like obstructive coronary disease. It requires symptom control and endothelial improvement.

Microvascular angina occurs despite normal angiography. Treatment usually includes beta-blockers, calcium channel blockers, nitrates, or ranolazine depending on symptoms and BP.

Dear Patient, Microvascular angina  occurs when the small blood vessels of the heart do not function properly despite normal major arteries on angiography. It is a real condition and commonly associated with hypertension, stress, endothelial dysfunction, or metabolic factors. Your normal angiography and lipid profile are reassuring. Treatment options (to be decided in person) • Continue blood pressure control — Telmikind (telmisartan) is appropriate • Beta blockers (like metoprolol or nebivolol) are often first-line to reduce heart workload • Calcium channel blockers (like diltiazem or amlodipine) may help vessel relaxation • Long-acting nitrates or Nicorandil may reduce chest pain episodes • Ranolazine is commonly used specifically for microvascular angina symptoms • Statins may be advised even if cholesterol is normal for vascular protection • Low-dose aspirin may be considered depending on risk profile Lifestyle measures (very important) • Regular walking or aerobic exercise 30 minutes daily • Stress reduction (yoga, breathing exercises, meditation) • Maintain healthy weight and avoid smoking/passive smoke • Good sleep and control of BP, sugar, and thyroid if applicable When to seek urgent care • Chest pain lasting more than 10–15 minutes • Pain with sweating, breathlessness, or radiation to arm/jaw • New or worsening symptoms Microvascular angina is manageable, and many patients improve significantly with proper medication adjustment and lifestyle control.
