I would like to know what's this condition called and I see it on my hair and need resolution asap. Would like to know the root cause. photo is attached.

It could be seborrhiec dermatitis or sebpsoriasis or scalp psoriasis. Try using and anti dandruff shampoo containing ketoconazole, salicylic acid combination for reducing the scales. For diagnosing the condition, further management,  visit a dermatologist
