If she was taking 125mcg maybe the doctor had suggested decreasing by 25 mcg and not to 25 mcg. You need to discuss with your doctor and dose needs to be increased again. She may require certain other tests too so do discuss with your treating physician especially if you are planning a pregnancy

This much Abrupt dose reduction should not be done . Repeat TSH after 4-6 weeks of change in Dosage . Other factors also needs to be considered like weight, dietary pattern, Other Hormones status, etc. as thyroid function can be improved in many cases.
Next Steps
Consult with details for further Management.

Does your wife had a swelling in the neck ( Goitre) when hypothyroidism was diagnosed ? If yes …. And if it got reduced after treatment with thyrox in high dose ….. And now they have suddenly decreased it so much low that , it’s too less of a supplement than what our body needs , hence TSH had raised
Next Steps
Contact me …. Because I need a proper history so that I can help you out for further evaluation … so that we both are not confused
Health Tips
Get the thyroid tests done on a empty stomach if not done earlier

Excessive TSH level. Dose reduction so abrupt is not justified. Dose reduction must be in interval of times. With regular follow up.

**Opinion:** Your wife’s recent thyroid function tests show significant abnormalities. The Thyroid Stimulating Hormone (TSH) is extremely high (77.9 µµ/mL), indicating hypothyroidism, while Total Thyroxine (T4) is low (4.13 µg/dL), further supporting this diagnosis. The abrupt reduction in medication from 125mg to 25mg likely caused these imbalances. It’s crucial to adjust her medication carefully to restore thyroid function. **Next Steps:** 1. **Consult Me Immediately:** Schedule an appointment to review her full medical history and current symptoms. 2. **Medication Adjustment:** Based on her latest results, her dosage needs to be carefully recalibrated. I recommend gradually increasing her current dose under medical supervision. 3. **Regular Monitoring:** Repeat thyroid function tests (TSH, T4) in 4-6 weeks after adjusting the medication to ensure levels are stabilizing. 4. **Symptom Tracking:** Note any symptoms like fatigue, weight changes, or mood swings, and report them during the consultation. Let’s work together to stabilize her thyroid levels—book an appointment for a tailored treatment plan. Early intervention is key to avoiding complications.
