I am 68 years old female suffering from type 2 diabetes for 24 years. I have done blood test on 13-09 -2023 . My creatinine level is 1.09 according to lab the range is 0.51-0.95 . GFR 55 and Hba1c 8.6 . Is it a matter of concern? How can I lower my creatinine and Hba1c and increase GFR ?

You are probably having diabetic nephropathy. Blood sugar needs to be in strict control to prevent further decrease in gfr. Need to be in regular touch with physician.
