Hi, based on the history you have shared, this may be a prolonged episode of your usual migraine-type headache, but because this episode is lasting much longer than your usual headaches and has not completely responded to paracetamol, I would like to make sure there are no warning signs before advising further medication. Please let me know: * Is this headache similar to your usual one-sided headaches? * Did it start suddenly and become very severe within seconds/minutes? * Any fever, neck stiffness, repeated vomiting, fainting or confusion? * Any weakness/numbness of an arm or leg, difficulty speaking, facial weakness, imbalance, seizure or unusual drowsiness? * Any blurring/double vision or loss of vision? * Does it become worse on coughing, bending or straining? * When was your last menstrual period and is there any possibility of pregnancy? * If possible, please check and send me your BP. * Also tell me exactly how many Dolo 650 tablets/paracetamol tablets you have taken in the last 24 hours and at what times. For now, please don’t take repeated doses of painkillers unnecessarily. Stay hydrated, have a regular meal, rest in a quiet/dark room and avoid prolonged screen exposure. If you develop a sudden extremely severe headache, weakness/numbness, difficulty speaking, confusion, fainting/seizure, persistent vomiting, fever with neck stiffness, or visual loss, please go to an emergency department immediately rather than continuing online treatment. Since this episode has lasted several days and is different from your usual pattern, I would also recommend an in-person medical assessment if it is not settling, even if there are no emergency warning signs.
Next Steps
Naproxen 500 mg PO at onset ± * Sumatriptan 50 mg PO for a moderate/severe migraine or inadequate response to simple analgesia.
Health Tips
Please rule out pregnancy before taking these medications

* Recurrent one-sided headache may be migraine, especially if similar attacks have occurred previously. * This episode lasting >1 day can still occur with migraine, as attacks may last up to 72 hours.
Next Steps
* Paracetamol 650 mg can be taken if required, keeping at least 4–6 hours between doses and avoiding other paracetamol-containing medicines. *consult a doctor regarding a specific migraine medicine such as a triptan rather than repeatedly increasing painkillers.
Health Tips
Maintain regular meals, adequate hydration and sleep; avoid known triggers. Contact me anytime on Practo for further consultation

Does it increase on exposure to light and sound. If so it must be because of migraine. If you also feel like vomiting, migraine must be your issue. Get proper consultation on Practo app or go meet a doctor personally.

Frequent one sided headache occuring monthly, could be a vascular headache. If its not relieved with dolo alone ,you can take a combination medicine like zerodol p for a day or two. However,if the headache persist, consult a neurologist in your area for a clinical examination and appropriate management.

A one-sided headache that returns roughly every month, and this time dragging on past a day despite your usual tablet, fits a migraine-type pattern rather than a one-off headache.
Next Steps
Keep a note of how often this happens and how long each episode lasts. That pattern helps a doctor confirm migraine and plan something longer-term than reaching for a tablet each time.
Health Tips
If tonight's headache comes back and the usual tablet barely helps, or you notice vision changes, weakness or vomiting, please consult a doctor instead of repeating it on your own.
