His sugar is very high. Please share details of his history then I can help him. He needs to take a sugar free, low fat diet, small frequent meals, avoid fruits till sugar is high. After sugar is o.k. , he can take fruits but very sweet fruits like bananas, mango, cheeku, should be taken sparingly. Regular physical exercise within   Limits of his stamina would be beneficial. Start tab metformin 500mg one just before meals. Do lipid profile,  repeat blood sugar fasting and post meal after a week of metformin.

What it means his sugar control in past 3 months is very poor. If it remains it can affect his eyes, kidneys, even lead to reduced sensation in arms and legs leading to injuries.
