Extraction with respect to 18 means extraction of your upper right wisdom tooth. Exact opinion would be easier to provide if there is an OPG( X-ray) or a clinical photograph . If the lower tooth is decayed or bothering you with pain or repeated swelling of jaw around that area or gums in that area or if there is a developing cavity in the tooth before that we advice extraction. If you’re symptom less and able to maintain normal oral hygiene you can continue keeping that tooth in the mouth
