The jelly-like fluid that comes out on straining is prostatic secretion being squeezed out by the raised abdominal pressure; it is harmless and not a loss of vitality, so please do not worry about it. The low back pain is usually muscular, from repeated hard straining. What actually helps here is not more laxatives but retraining. Please get an anorectal manometry with a balloon expulsion test - if it confirms dyssynergia, pelvic floor biofeedback therapy with a trained physiotherapist is the definitive treatment and works in a large majority. Alongside: fix your toilet habit - go at the same time daily after a warm drink, sit with a footstool so knees are above hips, allow only 5 minutes, breathe out and push the belly forward instead of clenching, and never strain repeatedly. Take 25-30 g fibre daily with isabgol 1-2 teaspoons at night in a full glass of water, 2.5-3 litres of fluid, and 30 minutes walking daily. Stop stimulant laxatives for long-term use; if you need something, polyethylene glycol (osmotic) is safer daily. Do see a doctor promptly if you notice blood in stool, black stools, unexplained weight loss, vomiting or a sudden change in stool calibre.
