This pattern suggests more of a chronic mechanical/postural thoracic pain with a myofascial trigger point, especially because:
pain is present for many years
it starts/worsens immediately on sitting and slouching
there is one specific tender point between the spine and right shoulder blade
exercise gives temporary relief
morning is relatively better, then pain increases during the day
Possible causes include:
chronic myofascial pain / trigger point in rhomboid or parascapular muscles
postural strain due to prolonged sitting, car seat posture, soft couch posture
scapular muscle imbalance
sometimes cervical/thoracic spine–related referred pain also needs to be ruled out if symptoms persist
Permanent solution usually is not one medicine alone. It requires correction of the cause:
posture correction during sitting
avoiding slouching and forward bending posture
proper back support in car seat and chair
targeted physiotherapy for rhomboids, trapezius, scapular stabilizers
stretching + strengthening exercises
trigger point release/manual therapy if needed
One medicine for temporary symptomatic relief:
Tab Paracetamol 650 mg after food, only when pain is more, up to 2–3 times a day if needed
avoid excess dosing
if you have
liver disease, consult before taking it
What I would advise next:
clinical examination by a physician/orthopedic/spine specialist
if pain is very localized and reproducible on pressure, physiotherapy assessment is very useful
if there is radiation, numbness, weakness, night pain, fever, weight loss, cough, or breathing-related pain, then further evaluation is needed
In most such long-standing cases, the best long-term relief comes from ergonomics + supervised physiotherapy + trigger point treatment, not repeated painkillers.
