It looks like caries approaching pulp.x ray is required to see the depth of caries.as such dont be scared of drilling as all procedures are done under local anesthesia

Hey There...Yes it is deep and you may need to get Root canal treatment done to save this little tooth. The drilling is not painful. Dentist will give you anaesthesia to make that tooth numb and so, you will not feel any pain.

Doesn't look major. But xray should be a better guide.. get the necessary treatment done at the earliest before the cavity becomes further deep...it could be done under local anesthesia if you are apprehensive about pain.

By seeing the pic v cannot gauge if it's deep.. an iopa wud be a better tool to diagnose this. From up it maybe small but spreads as a cone
