Back pain since 1 month

Hi, I am experiencing back pain since past 3 - 4 weeks, earlier I consulted a doctor who prescribed me a couple of medicines, I am attaching the prescription here as well, I would like someone to review of these are apt or will it take any modifications. Thanks, any help will be appreciated.
