In my opinion the following stepwise approach should be able to help you-
1. Comprehensive assessment- a detailed assessment of early childhood negative experiences and their impact on your psychological growth, assessment related to any maladaptive traits in the personality, current mental health state and environmental aspects contributing to the above.
2. Some basic medical investigation like
thyroid, hemoglobin, etc to rule out concomitant ailments.
3. Psychological tests- to understand psychological conflicts in a better manner (as per need based on above assessment).
4. Detailed plan of management- a comprehensive plan with well formulated goals of treatment and a collaboratively formed plan of action.
5. Psychological intervention- since you have received therapy in the past, it goes without saying that a psychological intervention would be central to your treatment and well being.
6. Medication- some medication can be helpful in controlling emotional turmoil and impulsivity, hence an assessment if medication are required and can be helpful should be examined in a scientific manner.
7. Regular feedback and update of plan- the plan will need to be dynamic in nature, will require to be periodically reassessed and goals be reformulated.
