Anal Fissure, sentinel pile & hard veins

Brother feel small pebbles like hard veins just on anus opening. While pooping , there is blood due to anal Fissure and there is sentinel pile. Currently on homeopathy medications. What to do to reduce the hard veins and sentinel pile?
