Hello Dear Don’t get too anxious it’s mostly Hormonal changes occurring as you transition into Puberty But if you still want to be sure then the most likely underlying issue is a hormonal imbalance, particularly Polycystic Ovary Syndrome (PCOS/PCOD) for which you should get a Ultrasound Abdomen and Pelvis done along with Serum Testosterone Serum Prolactin and FSH LH levels and consult a Gynaecologist . Watch for weight gain and get your Sugar levels checked However, other differential diagnoses must also be considered and ruled out like Hypothyroidism - do you have symptoms like Fatigue, weight gain, constipation, cold intolerance Hyperprolactinemia- this is a condition in which you’ll have absence of menses called as amenorrhea số get your prolactin hormone level checked Non-classic CAH ( congenital adrenal hyperplasia )- which is very rare bút can present as Early onset acne, hirsutism I.e excessive growth of facial hairs or over places where it’s not normal It can also be due to Stress, crash dieting which can also affect periods and hair so don’t stress so much and get your vitamin B12 Vitamin D and other electrolytes checked start taking Hair supplements if these are the cause of your Hairloss or consult a dermatologist for dandruff or any scalp irritation causing hairfall What you can do is Take Anti-acne treatments: Topical retinoids(only after consulting a Dermatologist), topical antibiotics like Clindamycin (avoid sunlight) Hair Fall management: Anti-dandruff shampoo, hair nutrients (biotin, zinc) Get a USG Done and Hormone levels checked along with serum iron and a CBC to rule out Iron deficiency Anemia which can cause irregular menses in order to preserve the iron store of the body Improve diet and take less salt less fat and decrease cheese or dairy products in diet for the acne Avoid touching your face too much as it can inoculate bacteria like Propionibacterium into your skin which clogs the sebaceous and sweat glands causing sebum production which causes acne Exfoliate regularly and use a gentle face wash after seeing a Derma consultant

Hi you need proper history and examination so kindly consult properly. It looks somewhat like a situation similar to PCOD by symptoms but few tests are needed
Next Steps
you can get USG abdomen done before a consult so that things can be more clear
