But as u said its spreading so it may got infected ,or itself it may be infected boil so u need antibiotics and supportive medicine to relive pain so consult any general physician

Can't come to a specific diagnosis with your history alone. I would advise you to meet a dermatologist so that it can be evaluated and you should undergo investigations to find the underlying cause of the problem and have it treated accordingly.
