Definitely removal is the last resort it treatment option. Depending upon the extent of cavity either filling or root canal treatment can be done. Nothing is painful. So don't worry

Removing a tooth is no more an option,treatment in such cases is endodontic management. If that can't be done,then only removing is a choice.
