Tears in Meniscus & ACL

Hello Doctor, I am from Hyderabad and came to know from MRI report that there are tears in my left knee at medial, lateral meniscus and ACL. Here, is the impression comments given in my MRI report:- 1. Oblique tear in posterior horn of medial Meniscus 2. Grade II horizontal tear in posterior horn of lateral meniscus 3. ACL appears bulky with intrasubstance hyperintensities, full thickness mid substance tear, retraction of torn ends and mild laxity of PCL. I am having 40 years. I approached 2 orthopedic doctors, the first doctor suggested to undergo arthroscopy surgery for ACL and both Meniscus repairs. But, second doctor said that surgery is required for Meniscus (either suture or clean of tear part) as my lifestyle is related to IT software activities and normal life style. Attached MRI report for your reference. Can these meniscus tears will heal themselves with out surgery? Please provide your suggestion on next steps.

Sciatic pain

Hello, i have Sciatic pain from last one year. Initially i tried to ignore it, thinking that it might be a silly cramp, but lately the pain is sometimes unbearable. In between, I went to the gym for 4 months, I didn't have any pain that time. I was feeling fit, and thought that the problem has gone. I quit the gym and the pain has started again. It runs down right from my hip to the calves( in my right leg). I sometimes have severe back pain too. I am not able sit in a place for more than 15 mins. Being in the IT industry I have to sit and work for long hours. I want to fix this problem. I am confused either to visit a physiotherapist or an orthopedist. Can you advice?

Right now the pain is reduced and swelling is also gone but still the leg is not strong enough to bear load, though i put little amount of load while walking with the help of walker.I was planning to rejoin the job in october( so far i am on leave), and want to continue my physio sessions in bangalore itself. Please suggest if its a good idea.
