When it comes to oral health, ignoring small issues like broken or decayed teeth can often lead to bigger problems. One such case we recently treated involved a young male patient who came to us with pain and swelling in his lower right jaw   a problem that had slowly worsened over the past week.

The Patient’s Concern

Our patient, a 26-year-old male, visited our clinic with a complaint of persistent, dull pain and mild swelling in the lower right back tooth region. He also mentioned an occasional foul taste in his mouth and discomfort while chewing, signs that pointed toward an underlying infection.

Clinical & Radiographic Examination

On examination, we noticed:

  • Retained root pieces in the area of tooth 46 (lower right first molar)
  • Mild swelling on the outer cheek near the jawline
  • Localised swelling inside the mouth near the gums surrounding 46
  • On gently pressing the gums in that region, pus discharge was observed, indicating a chronic infection.

To confirm the diagnosis, an IOPA X-ray was taken. It clearly showed:

  • The retained root stumps of 46
  • A radiolucent area at the apex, suggesting chronic periapical infection

Diagnosis and Treatment Plan

After evaluating the clinical and radiographic findings, our diagnosis was clear: chronic periapical abscess due to retained root stumps.

We proposed the following treatment plan:

  • Extraction of the infected root stumps of tooth 46
  • Prescription of antibiotics and painkillers to manage the infection and provide symptom relief

The Procedure

Once the infection was stabilized with medication, we proceeded with the extraction. The root stumps were carefully removed under local anesthesia using atraumatic techniques. The area was then thoroughly cleaned and irrigated with betadine and saline to ensure proper disinfection and healing.

Post-Operative Care

The patient was advised to:

  • Continue the prescribed antibiotics and analgesics for the full course
  • Follow standard post-op instructions including the use of a cold compress, maintaining a soft diet, and practicing good oral hygiene
  • Return for a follow-up appointment after 5 days to ensure healing was progressing well

Final Outcome

By the time the patient came in for his follow-up visit, the swelling had subsided, and there was no further discharge or discomfort. The site was healing well, and the patient was relieved to be pain-free.

Takeaway

This case highlights how neglecting old root stumps or untreated dental infections can lead to chronic issues. Fortunately, with timely intervention including medication, proper extraction, and good post-operative care such infections can be effectively managed.

If you're experiencing similar symptoms or have untreated dental issues, don’t delay. Early treatment can prevent complications and restore your smile pain-free and healthy.
