Warf dose after AVR replacement
My father INR reported today as 2.11 , He has diagnosed with Mechanical Aortic valve replacement 6 years back, He is doing fine, as INR range should maintain from 2.5 to 3.
What would be the new dose of Warf he should take in order to reach the INR range of 2.5 to 3.
Currently he is taking 3mg warf for first 3 days followed by 4mg for next 4 days in a week.
