Though it is very difficult to diagnose vertigo by short clinical history the history you are giving is suggestive of a condition known as superior canal dehiscence syndrome where patients will have constant imbalance, vertigo on exposure to loud sound, echoing of ones own voice etc. Diagnosis can be confirmed by high resolution CT scan of temporal bone and a test known as VEMP
