Uti or Something else. Is attached here?

I'm having problem from past 2months.First there was frequent urination, burning sensation and also sinusitis.i have taken oflox for 5 days and both got cured. But burning sensation and frequent urination came back after 2weeks ,this time taken niftas for 3weeks as per urine culture doctor prescribed. Today my urine culture and sensitivity reports are normal but symptoms still exist. Now sometimes there is pain in testes while urinating and burning sensation , also urine output is little but frequent. I'm attaching the reports (latest) Can anyone say what might be the issue. Old reports were KUB : NORMAL PUS CELLS : 7 EPITHELIAL CELLS : 8 ORGANISM : KLEBSIELLA (10^5) New report is attached here
