Urgent help. Ext or retreat 2nd step?.
I would like to know if we need to extract the tooth if retreating the root canal fails? Had root canal on a tooth 2 years ago and pain stays the same till now. No cavity or infection before rct. Tooth was hurting for tapping and biting pressure on it. RCT for same reason. Partial retreat in dec'21 distal root but no changes. Had retreatment again on whole tooth first step a month ago with intra canal disinfectant(CaOH) inside the tooth. Tooth is not healed yet and I am supposed to go back to the Endo next week to get the permanent sealing. Endo wanted me to decide on my own if I need to extract this tooth or finish the retreat. Cbct scans clear. Pain same since 2020. I need to know dental opinion if a tooth is not healed in a month with retreat disinfectant, does it need to be extracted? Cbct scans are clear but when Endo reopened 46 last month,saw mesial old filling wet&stinks. No pus. Blood color purplish. So he placed CaOH. Now he is telling to ext instead of 2nd step.

The endo seems well done. Cause of chronic pain might be due to a minor crack in the root which might cause this issue. Has the crown been removed while doing a second endo?

I would suggest removing the crown on the tooth and give the tooth time to heal for some more time.

The root canal looks fine.. The pain might be recurring if there is any fracture in the root.. A cbct would be helpful to diagnose.
