Tight anus

Asked for Male, 19 Years

Doctor I had an anal fissure for 3-4 months with a small sentinel tag that healed on its own but my spnector became tight .I went to a surgeon and he dignosed me old healed fissure and gave me ointments and laxative he told me surgery is not necessary.but my sphincter is tight I face difficulty during evacuation I can't clean my anus due to tight sphincter. What should I do doctor already said not to go for surgery everything is normal inside except tight sphincter is there any way to dilate anus to normal size without surgery.
