Surgery for lax cardia (gerd)
I am seeking advice on surgical options for chronic GERD. I have a diagnosed with loose lower esophageal sphincter (Lax Cardia) My main, most debilitating symptom is a constant sore throat caused by frequent fluid regurgitation. I have strictly tried everything—including extensive dietary modifications, lifestyle changes (sleeping elevated, not eating before bed), and medications—but nothing has helped a bit. Given that conservative management has failed, am I a good candidate for anti-reflux surgery (like a Nissen fundoplication or stretta procedure)? What pre-surgery diagnostic tests should I undergo next to confirm my eligibility? And how much these will generally cost?.

Sore throat caused by reflux of acid into the throat is one less common presentation: In your case, this seems to be the classical presentation. Confirmation is needed for surgical decisions making: Three tests are needed 1. Upper GI Endoscopy 2. Manometry ( impedance) to check for motility of the esophagus 3. 24 hour pH to confirm the cause and presence of reflux Once these are done and positive then an antireflux surgery is most likely to benefit you. Commonest surgery performed is “Nissens fundoplication “ although other variants also work well.
Next Steps
Get these tests done from a medical gastroenterologist and then consult a surgeon : surgical gastroenterologist

Pre evaluation tests required: * Upper GI endoscopy (if not recently performed) * 24-hour pH impedance monitoring (gold standard to document acid/non-acid reflux) * High-resolution esophageal manometry (to assess esophageal motility and determine the most appropriate procedure)
Next Steps
If testing confirms significant reflux with a mechanically defective LES and normal esophageal motility, anti-reflux surgery can be highly effective, particularly for regurgitation-predominant symptoms.
Health Tips
Need to visit hospitals and visit Gastro surgeons for the costs of the procedures.
