Suddenly eyes unable to focus , deep

I am  37 year old male , height 5'11 and weight 89 kgs , I am using tazloc beta 50 and CTD 12.5 tablets, From last 2 months when I am driving my bike suddenly I am unable to focus on the road, ( eyes deep pain) confusion in mind, unable to focus on road , , headache,fear , Nureologist given vartin -16 tablet. But this problem is not completely clear. I am unable to drive bike , Please suggest me why it is happening. My ENT doctor done sound test , ENT doctor said everything is good. My physiotherapist said it is a vertigo problem. 1)It is a vertigo problem - ear problem 2) nureologist ( brain nureo problem) 3)eye problem 4) with the High BP problem. 5) with the fear it is happening... I quit my job ,due to this problem , 😭 please suggest to me which specialist doctor give right medicine. Please suggest me. Please check attachment below 👇
