Sucide, sex , hydrocele, frequent

I am 22 years old student & unmarried.when I was 13 years old  i started the prone masturbation. After continuation of 2 years left the prone masturbation and starded with hand continue to yet (left a week ago). 1:-Now my penis has become weak,shrink and short size, size is 1.2-2 Inch in normal and 4inches at erection. 2:- My penis became thin and shrinkage and looks like a child penis.So I felt shy while bathing or urinating in front of people. 3:-My both testis(goli) became loose and right testis (goli) has became big in size and creating uncomfortable while walking, riding byke o, urologist,or sexologist etc . 4:-there is also problem of slow and frequent urination about 15-20 times in 24 hours. 5:-pls tell me what type of doctors specialist need I .e sexologist, urologist or general surgeon etc.6:-i want to physically meet with specialist in patna.please tell me what to do now
