Somwtimes gets pain in the penis

Hello my name is yaswanth ...from last 6 months my penis is falling after one ejection of sperms and for the second ejection of sperms it happens very less sperm count decreased and mainly sometimes I get pain in the penis ...the pain stands sort seconds and goes off ...it is happening from last 6 months ...what is the problem...I'm really afraid a lot...am I not able to deliver baby?...I'm becoming mentally I'll because of this ...please help me
