Skin problems whiteheads. Please help!

With these last few years i have got major or minor skin problems every now and then and i accept few things on certain level . But now its unbearable first of all if talking about face i have uneven skin tone whiteheads constantly on nose undereye n chin area i do clean up every 15 days or month but no use. I have lots of darkcircleseyes are tucked in and my face had sag in this age only upper lip and lower lip area are always black as i have . And then again in chest area near braline had done treatment of fungal infection 3 years back but no use its still red and scars near it. I sweat also lot . My skin type isexcessive oily or excessive dry . And every now and then i get somr infection near knee or some other parts of body leaving ugly scars . Please help
