Semen leakage. ejaculation problem,?

I am a frequent masturbator. I have a seminal leakage problem around 7 years..my erection and ejaculation also get worser due to this.I have consult a sexologist about my issues before 4 months.he studied me through complete blood test and Doppler scanning method..and he told everything is good but the blood flow to my penis only lower..he suggested a medicine for my issue..after took that medicine,I had feel another problem urine also get leaked..sometimes urine and sperm also combined..currently I have the problem such semen leakage with urine,cannot achieve 100% erection,shakey hands,my physical appearance also get worser,my penis size is small. Which doctor I could consult for my issues..?? Sexologists or neurologist..??
