With Att specially with ethambutol 20 mg pyridoxine (B6) daily is must to preserve vision &any optic nerve comlicationsin.

yes it can cause scotoma.moreover ethambutol in att itself may cause retinal problem.for precaution take tab or inj methyl cobalamine.
