Red blotchy skin on neck

My baby is 4+ months old. He develops red and blocthy skin on forehead after bath which dissaperas after a few hours. Now the redness occurs undereye and on nose too. I use Cetaphil soap for bath and Cetaphil cream as mositurizer. Water is not too hot either. Pics attached for reference.

I get red patches of skin on my arms and neck that if I scratch it , it will become raised, white and blotchy. Started suddenly about 3 years ago and it happens everyday, usually evenings and nights.
