The problem you are trying to describe could be chronic balanoposthitis. But it is difficult to diagnose just from your description. And if you are consulting a general physician for the same, then they will be of no use to you. Will need to consult a dermatologist who would probably send the scrapping for koh mount and fungal culture to confirm the diagnosis. And this condition is chronic in diabetics. Or he could look for any possibility of sexually transmitted disease if there is history of outside exposure. For now avoid using any creams, especially steroids. Avoid applications of solutions like dettol, savlon. Wash your genitals only with lukewarm warm containing little salt in it for about 2-3 times per day till you are able to consult a dermatologist for further evaluation.
