Penis tip skin is coming out. Me out.

Hii doctor, I had a fungal infection last year my doc suggested me for clopad gm cream and it was effective. 2 weeks before I washed my penis with soap and the reaction came again and the tip became red so I applied clopad gm cream again. It went and after that I noticed that whenever I masturbate of have sex with condom I face the same problem again my penis tip become reddish . Yesterday I saw some white substance on my penis tip when I washed it with water my penis tip skin was like coming out. I applied clopad gm again now it's feeling good but I am afraid will it come again. Please suggest me a medicine for this problem. I really need a solution because I am going to get married.
