Nature of my problem? Is my problem.

Sir/Madam,                                                  I have a problem on my skin on top of the scrotum in the genital area. My skin is itching so i am having to scratch the skin and because of that the skin has got cuts on it. I have used many topical steroids like fluticasone,picon,tacrolimus etc, antihistamines like cetrizine etc and I have also cut my nails short and also filed the nails. I am applying moisturising creams like nivea. My skin is gets dry if there is no moisture on it. Due to continuous scratching my skin colour and texture has changed compared to surrounding skin . I changed my clothing material also. i am facing a  lot of problems for the past 5 to 6 years or so and before that I had practically no problem and my skin used to be fine. I have visited many doctors and I feel nothing much has improved it became slightly better and after that it has become worse.I am also suffering from anxiety and depression. Can you please tell me what exactly is my problem.
