High inr increases your risk for bleeding which can be sometimes major and spontaneous. Exactly which anticoagulant/blood thinner your are on? Do you have any ongoing bleeding. You may need to stop it and repeat the inr test. But stopping the medicine, on other hand, increases the risk of damage to the artificial valve in your heart. So, first meet a cardiologist and show him all the available medical documents and get properly examined, so that the further course of action can be planned.  Hope this helps. Thanks.
