Looks like a small perianal abscess... You need clinical assessment with a per rectal examination by a surgeon! No one else ! A surgeon must do it .. and most probably you might not need a incision and drainage surgery but need antibiotics .. sitz bath and anti inflmmatory medicines ..

consult online if you can't meet a surgeon as soon as possible to get medicines alone ... but you definitely need a surgeon toget the per rectal examination done in person!!

Looks like abscess only …u have to consult directly for further confirmation …needs incision and drainage of the collection
