Yes, it harmful for anyone's health if they inhale smoke for a long time. In your case, as you said that you are a heart patient so yes it is harmful for you because small particles you are inhaling which are negligible,are harming your heart as well as your rest body organs. Hello Coal burning in open space is okay but still generated carbon monoxide which is not good for lungs - it might lead to death if you are inhaling for long period

Inhaling smoke for a prolonged period of time has health effects. Long term exposure to such air can even cause lung cancer. Short term exposure can lead to breathlessness.
